My Life Support - template for anyone
Serious illness, old age, dying.
There's a high chance you'll encounter these at some point.
And then...
Then it's very useful if your partner, family, friends, and caregivers can easily find your most important information. On one clear web page. Where everything is listed about the ailment, how you feel, appointments, needs, medications, care providers, results, documents, and so on.
At the end of 2021, I created such a page. My 'support team' made good use of it. Several doctors and caregivers also saw the value of it. They asked me to make the idea public and available to others. That's what I'm doing here. With a special template.
Some call it the "Death Book". I call it "My Life Support".
I didn't want to create a complicated website with all kinds of web pages. After endless experimenting, I chose Workflowy. It's a kind of 'outliner' and works approximately like this:

and this paragraph will appear, which you can also hide again using the arrow. This is called an "accordion". Accordions makes it possible to put a lot of information on one webpage – while still keeping it clear. As you can clearly see on this page and the About page

You can start with Workflowy for free. Then you can post a maximum of 250 posts per month. And thus gradually expand your information.
If desired, you can later take a subscription for $4.99 per month. The benefits: an unlimited number of 'posts', the ability to give others full or limited access to the whole or a part, or to let them read-only, automatic backups, and more. If you ever stop the subscription, all created posts will still remain. You can also add photos and documents - if necessary, secured with a password.
Just to be clear: I have no financial or other ties with Workflowy. I just find it incredibly handy, and now also use it as my favorite note-taking program. But those who don't like Workflowy can simply use the template (written in Markdown) as a starting point in another program.

Below are links to two ready-to-use templates. An English and a Dutch version. You can simply copy a template, and paste it into your own Workflowy. Then adjust it to your wishes: shorten, expand, change, and fill out what seems useful.
